All women who experience cyclical abdominal pain during and around their menstrual periods are referred to as "dysmenorrhea". From the perspective of Western medicine, some dysmenorrhea is primary, that is, no specific lesion can be found; and some dysmenorrhea is secondary, caused by specific gynecological diseases.

Chinese medicine, on the other hand, has broadly categorized the etiology and pathogenesis of dysmenorrhea into four categories:

1. Qi stagnation and blood stasis, mainly due to emotional discomfort, liver depression and stagnation of qi, obstruction of blood flow, menstrual blood stagnation in the cells to produce abdominal pain; it may also be the post-partum blood flow, storage and stagnation, and not through the pain, resulting in menstrual cramps.

4, liver and kidney deficiency, because of the ordinary body weakness, or more birth room labor, or after a serious illness, damage to the liver and kidney, qi and blood deficiency, the cells and veins of the loss of nourishment, does not honor the pain.

According to Chinese medicine practitioners, it is necessary to recognize the time, degree and nature of the onset of pain, and to distinguish between cold, heat, deficiency and solidity in order to treat the symptoms. Pain in the premenstrual or menstrual period, most of the real evidence; pain in the postmenstrual period, most of the deficiency; pain is more intense, most of the real evidence; hidden pain, most of the deficiency; stinging pain is more for blood stasis, distension of pain is more for qi stagnation; cold pain, heat pain, most of the cold, burning pain, heat pain, most of the heat evidence.

Etiology: The main pathology of this disease is poor qi and blood circulation, divided into two categories: deficiency and reality.

I. Empirical evidence.

(iii) Blood-heat stasis: inflammation of the uterus itself and inflammation of organs near the pelvis, prolonged fever, heat makes the blood boil, heat also injures the yin, and yin depletion makes the blood stagnate.


II, Void evidence.

Weakness of qi and blood, insufficient qi and blood in normal times, or damage to qi and blood after a long illness, weak menstrual blood can also lead to dysmenorrhea.

III. Points of defense.

(i) This disease is characterized by pain in the abdomen, and the dialectic is different because of the different times of pain; premenstrual pain is mostly a real evidence, menstrual pain is either false or real, and postmenstrual pain is mostly a false evidence.

(ii) Location of the pain: the middle of the abdomen for cold-dampness stagnation, both sides of the abdomen or one side, sometimes involving the back of the chest and hypochondrium for qi stagnation and blood stasis.

(iii) Nature of pain: stabbing pain for blood stasis, colic pain for cold; vague pain for deficiency, continuous pain for blood stagnation, pain stopping at times for qi stagnation; pain heavier than distension for blood stasis, distension more than pain for qi stagnation; heat pain for heat, pain reduction for cold; refusal to press for solid evidence, pleasure in pressing for false evidence.

(iv) Color of menstrual blood: pale blood is deficiency, purple is blood heat, and black is heavy heat.

As the old saying goes, nine out of ten menstrual cramps have stasis, that is to say, menstrual cramps are basically caused by menstrual stasis, and most of the people who have menstrual stasis have kidney yang deficiency, and most of these people are fond of heat and afraid of cold.
